[Mod_log_sql] mod_log_sql 1.100 Problems

Would you post your apache configuration where you are configuration
mod_log_sql (including loading the modules).

Add the directive

LogLevel debug

to your apache configuration and restart apache, then mod_log_sql will
give you more debugging information in the error_log
